Parts of the Ration Card Common Application Form and how to fill

The new ration card common application form for all digital ration card-related services has 7 parts. You need to fill out the different parts of this form based on the service you want to apply for.

Now let’s see each of these parts and what details you need to fill in each part of the common application form.

Part 1: Application

Part one of the form is mandatory for everyone. Here you need to select the services you want.

You can select more than one service at a time.

So put a tick sign beside the services you want for you or the members of your family.

At the end of part 1, you are asked if you belong to SC or ST category, if yes, select that appropriate box, otherwise leave it blank.

Part 2: Aadhaar Details of Existing Digital Ration Card Holders

Part 2 of the form is also mandatory for everyone.

Here you need to enter your Ration Card number, category, and Aadhar number of the members who want the Ration card service you are applying for.

Now besides that, you have to specify the “Service No.” from part one for each member.

For example, for ration card correction you need to enter Service No. as 4 in the box specified at the end of each row.

In case there are more than 4 members that need the service, take an extra form and attach it.

Part 3: Details of Applicant/Family Member

Part 3 of the form is mandatory for those who have selected Service No. 1, 2, 3, 4, 6, 8 in part 1.

Enter the complete details of each member who needs the Ration card service/services selected.

The details you need to fill in here include name, relation with HOF, Father/Mother/Spouse Name, Mobile number, Aadhaar number, Date of Birth, etc.

Part 4: Details of Present Address

Part 4 of the form is also mandatory for those who have selected any of the Service No. 1, 2, 3, 4, 6, or 8 in part 1.

In this part, you need to enter the complete present address of your family.

Fill this part correctly.

Part 5: Ration Shop Details

If case you have selected any of the Service No. 1, 2, 3, 4, 6, 8 in part 1, you need to fill up this part of the form.

You can get the name and number of your Ration and Kerosene shop on your Ration card.

After that, tick the checkboxes if applicable to you.

In the end, the head of the family should sign the application or put a Left Thumb impression in the specified area.

Annexure A: Auto-exclusion Ceiteria and Deprivation Criteria – Urban

You need to fill this part of the form as applicable to you.

Please make sure to fill this part correctly. The information filled must be true.

Annexure B: Auto-exclusion Ceiteria and Deprivation Criteria – Rural

You need to fill this part of the form as applicable to you. Select the appropriate option.
